What is Home Health?

Long term home health care is appropriate for anyone with a chronic health condition that they struggle to properly manage on their own. Skilled and licensed nursing staff come to the home on a regular basis to provide medical support and continued evaluation for setbacks or improvements.
Short term home health care is frequently prescribed when a person is discharged home following a hospitalization or needs temporary support with a medical condition. Our qualified case managers work with your physician and hospital discharge planner to ensure your doctor’s orders are followed and medical support is appropriate and beneficial.
Therapeutic services are often prescribed to enhance the recovery process and help patients overcome challenges. These programs help to improve mobility, increase independence and teach new skills that support safety and independence.
Who Benefits from Home Health Services?
Home health care is available for anyone who has a prescribed need for medical support long-term or short-term. It is most often prescribed following a hospital discharge, but at times, doctors will also advise home health to assist with managing a chronic health condition such as Parkinson’s, COPD, Alzheimer’s, a traumatic brain injury or diabetes.
Who Pays For Home Health Services?
